PORTLAND, Ore. (KOIN) — Oregon OSHA officials have confirmed that a janitor died Friday when he fell into a meat blender at a Clackamas plant.

CBS affiliate KOIN reports that the janitor was Hugo Avalos Chanon, a 41-year-old from Portland. He worked for DCS Sanitation Management, according to the Clackamas County Sheriff’s Office.

He was cleaning the facility at Interstate Meat Distributors, 9550 SE Last Road, along with several other people when he fell.

Despite the actions of a co-worker, who hit the emergency button, the meat blender could not be stopped in time.

KOIN reports that the blender is used to blend the meat and the fat.

The news agency also reports that this isn’t the first violation at the plant, which remains in operation.
